INTRODUCTION

Welcome to the 2007 Employee Satisfaction Survey.  We are conducting this survey again this year to continue to identify and meet the needs of our employees better.  Your responses will be combined with those of other employees and the results will be used as the basis for continued changes and improvements in the work environment at our company.

All responses will be kept strictly confidential.  Data will be compiled and reported anonymously, without reference to any individual.  Cumulative results will be shared with management and all employees.

INSTRUCTIONS

This survey should take no more than 10-15 minutes to complete.  Questions will be presented, by section (Sections I, II, III, IV, V and VI) and category (Categories 1-9, Narrative and Demographic).  Answers to all questions in a category will be required before you will be able to advance to the next.  The only exception will be certain narrative and demographic questions at the end of the survey, several of which will be optional.  They will be marked accordingly.  You may go back to review and change your answers at any time.  However, once the final "Submit" button has been clicked, the survey will not be retrievable. 

Our goal is to gather data from three key perspectives: 1) Topics with a broad, Company-wide scope, 2) Topics that reflect specific Functional Group issues and, 3) Topics affecting you on a Personal level. 

We also want your feedback on two aspects of each issue on the survey:

First:       Choose the level of agreement with each issue as stated
Second:  Choose how important that issue is to you as a NeuStar employee

Once you have answered all questions on a page, click on the "Next" button at the bottom of the screen to submit your feedback and move to the next page.
